I have removed the form as I found out this morning (April 2nd) that I wasn't receiving some of the requests. If you sent in a form and didn't hear back from me within 48 hours I didn't receive it. I always answer the same day or the following day. I apologize for any inconvenience this may have caused. I am working on solving the problem.
